Polar Express Hot Chocolate

Ingredients
1.5 cup, fluid (yields 2 cups whip Heavy Whipping Cream 6 serving Milk Reduced Fat Milk 2% Great Value ( 1 cup serving) 1 serving Sweet and Condensed Milk (1 14oz can) (by TLBLACKJACK) 32 tbsp Hershey's Special Dark Chocolate Chips 1 tsp Vanilla Extract
In a large pot over low combine all the ingredients. Stir regularly for 30-40 minutes until chocolate is fully melted and incorporated with other ingredients.
I choose to transfer my hot chocolate into a thermal carafe. For a party, you could also put all the ingredients into a crockpot a few hours before your guests arrive. Provide a ladle and mugs and let them serve themselves.
