Potato mushroom leek soup

Ingredients
4.0 medium (2-1/4" to 3-1/4" dia.) Potato, raw1.0 large (7-1/4" to 8-1/2" long) Carrots, raw2.0 cup Leeks1.0 cup, pieces or slices Mushrooms, fresh1.0 stalk, large (11"-12" long) Celery, raw2.0 cup Chicken stock, home-prepared4.0 tbsp Butter, salted2.0 cup Milk, 2%, with added nonfat milk solids, without added vit A0.75 tsp Salt0.75 tsp Pepper, black0.5 tsp Thyme, ground
Directions
Add all ingredients except milk and stock into a pot, and heat until butter is melted. Add stock and bring to a boil. Simmer 25 mins. Remove from heat and blend. Add milk and season to taste. Heat thoroughly and serve.
