Slow Cooker 2 Potato Soup

8 small potatoes, peeled and chopped1 cup chopped yam3 large carrots, chopped3 stalks celery, chopped2 med onions, chopped6 cups Campbell's Reduced Sodium Chicken Broth0.5 cup skim milk
Combine all ingredients except milk in slow cooker. Cook for 8-10 hours on low (or longer...). Pour all ingredients of slow cooker into blender and blend until everything combined. Put into large pot on stove and slowly stir in milk. Serve.
