This salad is FAMOUS in the town where I live, and so I have listed the recipe here. Of course, you could DEFINITELY make this a little healthier, but if you eat out, this is pretty much what you're getting. SO GOOD. Ingredients

Iceberg Lettuce (salad), 1 head, medium (6" dia) (remove)
Romaine Lettuce (salad), 2 cup, shredded (remove)
Scallions, raw, 6 medium (4-1/8" long) (remove)
Parmesan Cheese, grated, .5 cup (remove)
Romano Cheese, 4 oz (remove)
Provolone Cheese, 4 oz (remove)
Mozzarella Cheese, part skim milk, 8 oz (remove)
For the dressing:
Anchovy (anchovies), 6 anchovy (remove)
Cider Vinegar, 1 cup (remove)
Oregano, ground, 1.5 tsp (remove)
Pepper, black, 1.5 tbsp (remove)
Salt, 1 tsp (remove)
Granulated Sugar, 3 tsp (remove)
*Garlic Salt, 1.5 tsp (remove)
Cornstarch, .25 cup (remove)
*Crisco Pure Vegetable Oil, 2 tbsp (remove)
*Peppers, sweet, red, fresh, 1 cup, chopped (remove)
Directions
Blend all the dressing ingredients (except the red bell peppers) in the blender until smooth. Chop the peppers pretty small and add them to the dressing to allow the flavors to mingle.
You'll want the parmesan and romano cheeses grated and the provolone and mozzarella shredded. Chop the lettuce into small bite-sized pieces. Keep the green onions (scallions) pretty small, too, or they will be overwhelming. Toss the ingredients for the salad and add the dressing. Usually we mix the dressing and THEN serve, but you could do it the other way around.
I have calculated this for 8 servings, but if you used more lettuce and less dressing, you could certainly cut out many calories per serving.
